Sput, I can't remember but I got it from PirateJack out of Asheville, N.C. 2010 I paid $169 for it.
The number is PBUFMCT4754 to check it out. I think their web site is Piratejack.net

It came with brackets that bolt to chassis along with pedal. Nice set up.
