What Carl said. When you take it back off, check to be sure the disk will slide freely on the transmission input shaft. If it hangs up, it can make the clutch still turn the gears when the clutch is depressed enough that the gears will grind when shifting. Putting washers under the pressure plate is just a bad idea band-aid.
