Received my order from JC, and opened the cab mount kit. It contained 8 bushings, two rubber squares with matching steel plates, 2 cupped round disks, and 2 bolts. Question - since none of these existed when I got the truck, where and how do they go. I am assuming the square rubber and steel go on the back shackles, not sure about the bushings, and assume the cupped stamped disks go in the front somhow, not sure how or where. Also I am not sure why only two bolts,and why is there nothing for damping up front?

This is how it assembles:
The round rubber bushings go in the rear mounts 4 in each one. You can see a completed refurbishment of one here (painted red because I like it factory are usually black under the undercoat).

The piece with the bolts goes through the rubber then you put on the additional plate and tighten so they compress a bit. There are no rubber pads here some trucks are shimmed with small thin metal shims on either the frame or the cab side. The thicker metal threaded piece goes inside the cab behind the seat pan.

The two rubber squares go in the front between the cab and the mount on the frame, the steel pieces go under the frame mount where the nut and washer tighten, the cupped piece goes on your cab floor and the bolt goes through it. You should really consider buying a Factory Assembly Manual for these trucks that most vendors sell since it has good views of these items.

Thanks, I find the assembly manual confusing at times. I now know that I have bushings for the shackles, and the rest of the hardware goes to the front mounts. I don't have the plates you show in your photos, either threaded holes or through holes. Would it be possible to give me the thickness of those plates and the bolt lengths, I assume the length and width of the plates corresponds to the shackle.
I can have the shop at work make something up for them.

Yes I will measure them up tonight and get the details to you on Monday. They are best to use for the cab mounts because depending on strain washers and nuts can pull through the floor. The shims are just for leveling the cab if neccessary at the factory based on measurements but you may find you need them so I will measure everything up.

Ok here are the numbers for you:

Cab locking plates are 1/4" thick by 1" x 2-1/4" they are threaded for 3/8" NC bolts which are 1-1/2" long the holes are 1/2" in from the edge on the long-side. The shims are 1-3/4" x 2-1/4" with the holes the same from the long-side they are 15ga material but anything close would work. The bolts that hold the bracket to the frame are 1-1/4" long 3/8" NC.
