They're called "companion cylinders". One is on compression while the other is on exhaust, on the upstroke, and they run intake and power strokes on the way down- - - -exactly 360 degrees apart in the firing order.
Inline sixes run smooth because the three cylinder pairs are exactly 1/3 turn apart- - - -naturally balanced. 4-cylinders, V8's and V6's are trying to shake themselves to pieces all the time.

How many of us complain about our V8's, our Honda or Toyota 4 cylinders, our 90 degree V6's? And now we're seeing new 3 cyl. engines. They all run well and smooth, whether by balancing or proper engine mounting systems, and they live for 200,000 miles these days. I think BMW is doing a PR job trying to justify their inline 6 engines. They also sell V8's and other configurations.
