I have often thought of a 265 with a throttle body off a 4.3, electronic ignition and a 700R4 trans and a different ration rear end. Another I was thinking of was a 4.3 with a throttle body, 4l60E transmission, and a rear end. The 4.3 setup delivers 17-21 mpg on my 1994 1/2 ton.
