If you run into one that will not come out with the mechanical puller or "grease" method I've found that using a center punch to mark a spot and a 1/8 drill bit will relieve enough tension so that it can be mechanically removed.
The key to getting it out with grease is to pack it full....I use a 5/8 wooden dowel trimmed down to fit tight in the bore and smack it SMARTLY with a 3 pound+ hammer. I have a good pilot brg puller and it doesn't always work. Hope this helps.
