The most common clutch installation mistake is putting the clutch disc in backwards. The disc hub with the torsion springs in it has about a 1/2" offset, and if you put it in the wrong way, the springs contact the flywheel attaching bolts and won't let the disc release all the way. The thicker side of the hub needs to face toward the transmission, away from the flywheel bolts.
