A worn out pilot bushing can cause all sorts of problems...including the ones you mentioned. When the clutch is disengaged completely, the input shaft of the transmission can sort of wobble and "walk" around there and make a racket. Especially if your bushing has worn to an oval shape. And it is worse during a full moon.
