It's totally doable and pretty easy if you keep the wheels and tires both 22.5". The adapters are easy to install and made to order. They can be purchased from numerous places, but ET is the place I got mine. They run $1200 for a full set with all lugnuts for the big circle included. They are pretty heavy duty and weigh about 35lbs per adapter. They are very stout.
You simply bolt them to the truck with the inner track and then bolt your wheel to the outer track and install whatever lugnut and hub covers you choose to install, if any. The only reason I went the route I did is that I wanted a hot rod look for my bus with the low profile tires and the commercial tires were too tall for what I wanted to do.
