To date, have not had one fail in service.
When rebuilding the engine, I would always replace ALL core plugs (as Kevin mentioned, freeze protection is a secondary function).
For a driver for the convex plugs, I would start with a piece of brass bar larger than the diameter of the plug, and turn down the end on the lathe to be slightly smaller than the diameter of the core hole, and flat on the end. This allows force to be applied in the very center of the core plug, and gives the best expansion. And no, I don't use sealer.
I use the same method on carburetor core plugs.
