It may just be a carboned up injector not spraying a good pattern or it could be carbon on the back of the intake valves. If it runs relatively smooth, add some injector cleaner additive to the gas tank and drive it enough to heat everything up. It will take a while if your truck does alot of short trips or stop and go driving.
