Replace that fan, check it close for cracks and signs of breakage. Also check the pulley the belt rides on. Take them both off the water pump and look them over very closely. I had a fan blade break and come apart one time. Luckily no one got hurt, but could have if they had been leaning over the fender with open hood and engine running. Also, check the input shaft of the water pump for bad bearing that may have let the blade wobble around.
