I'm not too familiar with the clamshell door but I will assume the glass is flat.
If so any competent glass shop should be able to cut one for you using laminated glass. If you prefered tempered glass they should be able to get one made for you.
Maybe someone will chime in and confirm if the glass is flat or not.

I crated them up and stashed them in the attic. Everyone says the clam doors are the ones to have, I personally like the looks of the barn doors. Inuke

I took a picture of my hinges today so that I could post, but they are bad as yours! This is typical of many of the clamshell hinges that I have seen.
