Is it safe to say if I install a 12 volt starter , 1954 261 engine, I must have a 168 tooth ring gear?

Yes, if you install a 12 volt starter you MUST use a 168 tooth flywheel ring gear.

Look HERE for details.

I've had a 6 volt starter and flywheel on my converted to 12 volt electrics truck for over 40,000 miles.

I believe there's a different inside diameter ring gear between the 139 and 168 tooth gears. Just put 12V to the 6V starter and move on. If you want to have the resistor bypass terminal on the foot switch, just use the 12V switch. It's a bolt-on swap to the 6V starter.
