I have a few stovebolt manifolds that have one of the ears for the exhaust pipe attaching studs broken off, but with a properly functioning heat riser valve. Simply trying to weld a stud into place usually doesn't work very well, but I think I might have come up with a better fix. How about fabricating a flange from 1/2 inch steel plate, and attaching it to the existing flange by brazing or nickel-rod arc welding, and threading replacement studs into the new flange? The repair part could either be machined for a flat gasket, or tapered for the newer style donut gasket, depending on which type of seal the engine builder chooses to use. A pretty competent welder could add a second flange and a blockoff plate between the front and rear exhaust ports to duplicate the scarce and expensive "Corvette" twin pipe exhaust manifold at the same time. I don't have any parts to look at. I may be way off of the area in question. But here is a try. Feel free to Kibosh or embellish. Actual shape of threaded plate TBD. Flat gasket will seal. This will look ugly with the hood open.

If your welder lacks the competency to build up and re shape the ear in preparation for facing, drilling, tapping, and beveling I'm certain he lacks the skills to add another outlet. I made several split manifolds in both 1 and 2 pc. style and made a fixture to automate the process of shaping the pipe to the manifold in minutes. When one gets too many projects on their plate some things get pushed to the corner. I have been hanging on to 5 pipes I prepped for welding for over a decade now and they're in danger of heading to the scrap bin soon so if any bolter wants one send me a PM and it's yours no charge.
I agree on the "ugly" part. Since I've been brazing/welding cast iron with a fair degree of success for over 40 years, I'm a little more comfortable with adding a steel flange under the existing one. The steel will also be more durable and not prone to breakage like the cast iron flange can be. If the manifold gets powder coated after the repair, it should look pretty good as well as being functional.
