The PTO is forward facing for a winch. Problem is that the bell housing cross member is in the way. The 30 degree adapter re-positions it so the line line passes underneath. The PTO appears to be a Dana/Spicer(from research). I'm trying to identify the adapter now so I can attempt to locate one. No luck so far.
That PTO closely resembles one I had on a heavy haul wrecker. It powered a 35 ton belly winch, and it had forward and reverse direction capability. I believe the fwd and rev ratios were slightly different due to different tooth count gears inside the PTO. I don't remember a PTO brand name or part number- - - -I managed that wrecker service in the late 1990s.

Another possibility is Chelsea. They were/are a manufacturer of PTOs.
