If that horn was on your 1949 truck, it is not the original horn.

My 50 has a horn similar to yours mounted to the boss on the intake. I do not know if it is original but I believe it is a period correct horn

The mount is diffrent than yours and it does not have that additional item on the bottom

To test the horn just ground one lug on the side of the horn body and power the other

...example...

Im 12 volts and added a relay, not sure how the original one is wired.

I will try 6 volt to horn first, hopefully it is good, can it take 12 volts,or should I reduce?
I was wondering what those 2 holes on intake were for.

The two holes on the pre-1954 intake manifold were for the standard-equipment horn (pre-1953) and (1953) .

You can put 12 volts on it for a test no problem.
Just rememner to ground the other lug to your source.

If you have a shop manual the wiring diagram is in the back of the book
There is one on line but I cannot read it.

The FAM shows a different horn, so I couldn't rely on that
I will look at shop manual

I will try directly to horn (when I get back to it next Wednesday)

testing with Bat charger, so I can switch 6 to 12

thanks for your help, if it works I will make a bracket to mount to boss on intake

the pre53 looks a lot like my horn

you can run 12v on a 6v horn no problem. It might burn it up if you were to lay on the horn for a few minutes straight but, who does that? I ran 12v through my 6v horns in my DeSoto for years. Run 12v, not 6 as a test. You may also need to use percussive maintenance on it while applying 12v (give it a whack with rubber mallet or something)if something is stuck inside, that should jar it loose. Yo may also grind the rivets off and clean it up inside (I think there may be a pair of contacts inside that can corrode). Yo can replace the rivets with small (looks like 10-32 bolts) or use pop rivets to put it back together. That extra little box on it can probably just go away, it may be a relay but,

There was a horn relay with the 1953 and later "seashell-style" horns (and the accessory "seashell style" dual horns option 1947-1955st).

No relay was used with the single standard horn from 1947-1952.

I have gotten some to work again by applying power and giving them a good whack at the same time (coil gets stuck and won't vibrate). I know that is not a very precise way to troubleshoot though! I have also ended up tossing some (or letting them sit in my shop for 15 years thinking one day I will learn something more and fix it!)
