I think I got it out (and back in) from underneath, with the truck/frame jacked up and resting on high jack stands.
Do not forget to take out the "hidden bolt" that is underneath the box/frame (and often covered/hidden with caked-on grease/dirt).

I have it all loose ready to go. The hard part was getting the pitman arm puller on to pull the pitman.
