Also, the other side looks like someone took a hammer to it and broke it: Pic of Yoke I've searched my normal replacement part sites but was unable to locate a brand new yoke. Any suggestions for me?

You might try a shop that specializes in rebuilding driveshafts, as they will have sources the average parts store won't know about. It's also possible that one might be listed on Ebay, but be absolutely sure the diameter, length, spline count, etc. is correct before ordering one from somewhere before doing a side-by-side comparison. Fortunately, there's a pretty good bit of interchangeability on slip yokes, sometimes even crossing over car lines- - - -GM, Ford, MOPAR, etc.

Lots of time people have something listed by p/n and have no clue as to what it fits. The best would be to find the part number. I have all GMC and don't know much about Chev. It will probably be the same as a GMC and Chev and GMC use the same part numbers.
