They're are kits available that place the m/c about a foot away from the pedal assembly as Joe(Zookeeper) mentions,but as yet,I haven't seen one for an AD truck.
I'm running an auto trans so haven't had to deal with this problem.
Here's one i've seen for an earlier Chevy,I'm not sure if it could be adapted to fit.
It will give you the general idea though.
