A friend with a lathe can come in handy here. Turn out a piece of round stock that's a slip fit in the old bushing, with just enough shoulder on it to catch the edge of the bushing. carefully push the old one out with this tool, and push the new one in the same way. Be sure to leave enough clearance on the inside that the new, tighter bushing doesn't sieze up on the driver as it's installed. If the bushing is in a blind hole, use something like a Dremel tool to cut a thin spot in the old one and use a cape chisel to cut it out.
