Okay, sorry this took so long. Had to wait for daylight to crawl under the body. ( And after the kids were fed, etc.) Starting at the rear. Rubber pads and carriage bolts. Mount just behind the axle hump, blocks, rubber pads ( between the frame and block ) and carriage bolt. Mount just in front of the hump, block, rubber pad and carriage bolt. Mount behind the door, rubber pad and carriage bolt. Mount at the very front, rubber pad and cap screw ( hex bolt.) You don't have anymore then 10 pads, do you?? You can also get the offset washers for underneath the carriage bolt heads. Looks like Chevy Duty has them in zinc, stainless and polished, although I think the plywood takes the cup type and the Chevy Duty ones look flat.
