I used a spedi-sleeve to fix mine that had huge grooves in it. It worked great!
A local parts house should be able to help you choose the right one. (if not I think that I saved the old box from the one I used - I can try to find it).
A tip on installing it: Boil your sleeve in water and stick your yoke in a bucket of ice water - this way when you install the sleeve it slides on pretty easily. Once they equalize in temp it is on to stay!
