If you're pulling the tranny with the engine you need a load leveler. Like the others have said, On these trucks it's usually easier to pull them separately. If you have a floor shift box, you can pull the floor plate and use an engine hoist through the passenger door to lift the tranny.
As for bolts, I usually use a front and rear head bolt. Sometimes I use the front manifold bolt and the rear coil mount bolt. Don't lift by wrapping around the manifolds, they crack too easy and don't need your help starting new cracks.
