I think it is a 'custom' fix. I had the same thing on one of my window handles. I removed the set screw and the handle would not come of. Using a bit of leverage the handle popped off breaking the splined shaft on the regulator. There was a nail being used as a pin. My other handles (both trucks) have a pointed set screw, the point goes into the groove in the splined shaft. I also believe that there is another chrome screw which goes in the hole on top of the set screw to either lock it in place or add a finishing touch. I have only seen one like that but am told they are supposed to be there.
