It's a lot more likely that during a valve guide replacement some time in the past that the guide was installed a little bit crooked, or maybe the guide is badly worn. What's the compression on that cylinder compared to the rest? Which valve is it, intake or exhaust? You can check for a worn guide by packing the cylinder full of small-diameter rope to prevent the valve from falling in, turn the piston up tight against the fill rope, remove the valve spring, and check the guide for wobble by shaking the stem from side to side.
