Drill and tap the cam gear for a crowfoot puller like the one used for a harmonic balancer. If the cam gear is the fiber type, just break it off with a pry bar. Once the cam gear is off getting to the thrust plate bolts isn't a problem. PB Blaster or Kroil in the cylinders, followed by heating the piston crowns with a torch will usually free them up enough to turn the crank. I like to use the chisel end of a shepherd's crook wrecking bar between the flywheel teeth and the bellhousing to help turn the crankshaft. Once the pistons free up a little, things usually move along pretty quickly.
