I had similar issues when I first got on the road,the initial problem was caused by slippage which 'glazed' the belt,it was all downhill from there....
I had painted my pulleys during the rebuild including the 'V' grooves which I think initiated the slippage.
After sanding out the pulley grooves to bare metal (which would ordinarily happen over time anyway) and replacing the belt with another new one i've never had the problem since.
