Probably a bad throw out bearing.
I have a brand new one on my work bench which is so bad it osculates when you turn it by hand! It is not loose, and I cannot understand how it could have been made that way by accident.
The Chinese government currently has all of my personal information (due to sloppy US federal bureaucrats), so maybe the Chinese flagged my order for a bad throw out bearing.
