The hoses come off the pump which is mounted to the wiper motor itself (which is under the dash). I do not have the routing diagram for the hoses but one sucks the washer fluid to the pump and the other tow outlets go out to the firewall and up to the nozzles

If you talk to the right person at a parts store you might be able to get a sample (inch long or so) of each size of hose for a test fit.
