I'm having trouble describing this, but the offset washers that the long bolts go through on my '48 half-ton need to be set up, and I need to know, in terms of the hands on a clock (from the rear view of the bed), toward what number does the largest side of the washer face? At 12, toward the cab?

You are correct, the large side of the washer goes toward the front of the truck. http://www.mar-k.com/mtgholes_3-8_ofst.pdf Check figure 4 step 4 for a diagram. I expect it would work either way, but the original trucks I have seen are like the diagram.
