The reasons I have heard are 1)old laws that stated you were not to exit the vehicle into traffic 2) the gentleman was to unlock the passenger side for the lady and she was suppose to be polite enough to open the driver door for him.
Just what I've heard.

Much safer to lock the big bolt from the
curb than from the traffic side...don't want to get hit by an old Ford or International. Good old fashioned thinking from Detroit.

Push the inside handle down, scoot out the passenger door and lock with the key!

The driver side door is locked/unlocked from the inside. There is a 3/16 inch square shaft near the rear of the door, and the "T" handle goes on that shaft.
