They say to slip a washer over the bit to stop you from drilling the hole too big. But since the bit is conical, you will end up with a hole smaller than the washer's hole. I suppose you need to drill out the washer a little in order to get the desired diameter.

Right up there with a "fits-all", "duck tape", screwdriver pry bar and a big "make it fit" hammer.
The step drill is used a lot for making holes in electrical panels to fit conduit and Romex connectors.
