Looking for repair recommendations for rust through at top of center rear window frame of a 54 AD truck. should I cut out and make patch from a donor? I've not found any patch panels for this area. Thanks for insight/recommendations...

I've not heard of rust in that area Greg. Since it's very
unusual for an AD to rust out there I doubt anyone is
making a repair panel.
This is a case where one would have to hand fabricate
new sheet metal to patch it.
We don't know your capabilities but if you don't have the
skills to tackle this sort of task, then you might want to
check in with the body shops in your area or maybe a car club
to find someone familiar with fabrication.

Depending on how big the area is, it might be easier to cut out the bad spot and hand-form the patch from scratch.

Depending on how much rust, and what your expectiations of the finished product is, you may want to remove the whole thing.

Simply drill out the spot welds "unbolting" it. Click on the link for the "Basics of Basics" of doing this. http://www.hotrodders.com/forum/basics-basics-spot-welds-removing-welded-panels-222549.html

Click on the link to see a photo of one removed.

Then you just plug weld it back in when done after the whole inside is protected with Epoxy primer.

Took me a while to find it but this old thread from MP&C has good info for forming panels.

Its been my experience that when its rusted through in that area its because of a rodent nest and its also very near rusted through the other sides well. They do make repair panels but its the whole back upper panel. Poke the panel along the edge with an awl and see what poke through. it will tell you how far you need to go.
