I suspect the speedo cable needs to be replaced. I am wondering if a light truck cable will fit a medium truck. It has a five speed in it. Check the cog at the end of the cable where it enters the gearbox, is it the correct cog? (might be colour coded).

Does it work but just isn't accurate or is the needle bouncing or what??? A little more info will help us diagnose the problem. Sometimes it reads what might be correct, then it reads zero, then it reads less that what makes sense. I suspect the cog is correct as it passed a commercial vehicle inspection not long ago. They're usually made of plastic or phenolic and the cable end tends to wear the square hole enough to round it off. You could pull the cable out and inspect it, but I'll bet you find it's still square.
