As best I can tell from your photobucket pictures, you have the correct top hinge. Perhaps you could put a better and closer picture on your photobucket for us to see. I think the service manual shows a standard cab, which has a different top hinge than your COE.
Both of the hinge "straps" inside the door have slotted holes. These allow you to adjust the door in the opening. The top hinge is behind the cardboard panel at the top of the door. The bottom hinge is accessed after removing three hole plugs(which is very different than a standard door). These plugs are often missing and you may just have the uncovered access holes. By gently loosening the bolts you can tweak the alignment of the door.
