It would be best to drill and tap holes using a machine. A guy that knows his stuff could hand do it. The only way an assembled engine could be machined like that is either on a good size old horizontal milling machine or horizontal boring mill. A nice $300,000 to $500,000 horizontal cnc can do it too, but no shop will have one with enough free time to mount it too the table though.
