International manufactured the "skinny cabs" until very recently, and may still manufacture them. I see them around San Antonio quite often.
I saw one a few years ago in a yard not too far from where I live--not sure whether it's still there or not. It was a mid 50's vintage.
In addition to the regular doors, they also had a trap door in the roof so you could enter and exit the cab with a load on.
