Maximum vacuum at idle is the maximum vacuum reading you get as you slowly rotate the distributor in both directions.
You first set the idle speed. The idle speed might increase as a result of the above operation. So, you may want to reset the idle speed and repeat the operation in the first paragraph, above.
Attach the vacuum gauge to the wiper-motor vacuum-port on the intake manifold.
