This is how it assembles:
The round rubber bushings go in the rear mounts 4 in each one. You can see a completed refurbishment of one here (painted red because I like it factory are usually black under the undercoat).

The piece with the bolts goes through the rubber then you put on the additional plate and tighten so they compress a bit. There are no rubber pads here some trucks are shimmed with small thin metal shims on either the frame or the cab side. The thicker metal threaded piece goes inside the cab behind the seat pan.

The two rubber squares go in the front between the cab and the mount on the frame, the steel pieces go under the frame mount where the nut and washer tighten, the cupped piece goes on your cab floor and the bolt goes through it. You should really consider buying a Factory Assembly Manual for these trucks that most vendors sell since it has good views of these items.
