the 58 is indeed 4 eyed
the low beams are the outer pair
the high beams should be all 4 [same bulb #s]
there is a thermal protection circuit on the light switch
in a quiet place, engine off, turn the headlights on, click to high beam, listen carefully for any click noise from the switch
could be a poor connection heating up, which could be the socket that plugs onto the bulbs - if so, you might see some evidence there in the form of melting plastic
