The lack of something, possibly a thrust washer, is causing the excessive gap between the gears that you mentioned. One scenario would be that hitting the clutch a second time causes backlash of the gears and the second gear is moved into a better position. Clutch adjustment can have nothing to do with anything inside the transmission, as the only thing the clutch touches is the input shaft.
