I suspect the middle part of the axle gets bent forward due to hard impact(s) on the wheels. The front springs would act as fulcrums, forcing the middle of the axle to bow out in the forward direction. As others have stated, king pin angle is the most important dimension in a beam axle, as toe-in can be set with the tie rod.
