I recall seeing jacks like that years ago, but don't have any information other than I believe the handle was a flat bar that was also use for demounting tires (was spoon-like on one end). I think there may still be one of those tire irons in my mom's garage that dad had and I used when I was in high school.

A quick search on eBay shows a number of similar ones, calling them Model T Ford jacks.
