I have seen a hub spacer just as you describe when using the 19.5" wheels, would take some quality machine work to make them, I thought the lug nuts would seat it centered fairly well. I have a 2 speed rear in the 39, it is cable operated, but has the gearing normally associated with >=47 2 speed rear end, not <=46 gearing, I really want to test fit them.
