Have a look at this Budd lugnut and stud diagram

The "second lug" is the stud looking part with square end on it.

To remove dual wheels with this mounting system first remove the big nut looking lug nuts, then outer wheel falls off. Lastly remove the stud looking inner lug nuts with square heads, and inner wheel falls off.
