I had the same question. I couldn't find the answer so I ended up taking a large pin with a head on it that had holes drilled in the sides of it and tapping it. Then I used some washes and put two jam nuts on the end. It worked, but I'd rather have it the way it's supposed to be. I saw a picture of one on youtube that had a bushing in the hole on the arm and a pin through it that had a hole in it for the cable and a nut on the end of it.
