Looks like an old Perfection pump made in Galion OH and has been out of business for many years. These are low pressure gear pumps meaning 1000-1200 PSI and were about 12 GPM@1000 RPM. It would help if you supply a photo of the hoist it operates if that is what your truck has. Perfection farm hoists were twin telescopic cylinder types and used an O ring seal. Not terribly good cylinders either I might add but the pumps were OK and equal to the other national brands. They (as did the competitors) also supplied pumps to other builders of truck mounted equipment so it wouldn't necessarily have to be a Perfection hoist. I sold a ton of them when I was a dealer for them and had little trouble with the pumps.
As for seals just find a seal specialty house on the internet and measure the shaft size for the packing ID and the stack height. It probably has about 5 pieces. There are MUCH better seals available now than the originals. Should NOT be a problem to get seals for it or much of anything else. If you pull the front & rear covers of it to check the wear plates you'll need to replace the thin paper gaskets with the exact same thickness or the pump will loose pressure and not work well. Worn bronze faced wear plates will allow slippage past the side of the gears. If yours still pumps well then leave it alone and just deal with the leakage.
