I assume that the lock is stuck, but I cannot figure out the mechanism on the inside of the door. Does anyone have an pictures and or advice to get the door open?

What happens when you pull the outter handle, assuming you have lever type handles? Or has someone changed it over to push button? I would spray some WD-40 around the latch area and give it a push pull/jerk ect. It could be anything from the latch not working properly to the door being so badly out of alignment that it's jammed shut.
Don't pull on it from the outside. From the inside, pull the door handle and bump the door real hard with your shoulder. If it still won't open get out your tools and get in there and figure it out.

You'd to have some support in place so the door doesn't fall out but rather is removed in a controlled way.

As I understand his question its not the door that is stuck, but the latch. Undoing the hinges at the very least will allow jiggling the latch and it may allow removing the door without operating the latch. Those old latches don't hold all that great in the first place, it shouldn't be real hard to rattle the door off with the hinges unbolted. But I'll admit I haven't tried this approach. Last time I had the problem I spent weeks spraying the latch with penetrating oil and eventually was able to operate it. I was in no hurry so it didn't really matter.

I would suggest cutting the cardboard cover off the inside of the door to see if the remote handls is moving the lock. The latch has to be addressed. If the truck has the cardboard cover on it, it needs to be removed. If the latch will not operate by way of both the inner remote handle and linkage, and the outer door handle, then there may be a serious problem with the latch, and it may need to be replaced. Get inside the door with a flashlight and your fingers. See if you can figure out, or force the latch to release. They are a simple mechanism. I ended up pulling the seat from the passenger side (car not truck but should work the same) using access from the passenger side, pull the door trim panel then work on the guts of the door.
