Jack -- I got mine for my '49 3804 from Jim Carter a few years ago. Here;s what mine looks like installed.
BTW -- I know Jim Carter personally and have enjoyed the pleasure of his personal company on multiple occasions. You could not be dealing with a better supporter of our web site!
Here's some pics I got tonight of my application. I hope you find it useful.
Regards, John

John, you might benefit from a couple of rubber boots on the levers. Keeps the heat out in the summer and the noise out all the time. Unless, that is, if you're going for un-restored original. Some merit to that, of course.
I stopped by Jim Carter's booth at the ATHS truck show a few years ago and talked to him for most of an hour. Very nice guy and very knowlegable. Ordered some parts on the spot. They got home before I did; maybe because I was driving from Colorado Springs to Seattle in a 59 Kenworth pulling a flatbed loaded with a 41 GMC 2-ton and a similar Mack A-model. The cops at the Wyoming port of entry got wide-eyed as we rolled across the scales at 3 MPH with that load, "Park - Bring Papers" sign flashes. They just wanted to look at the old trucks.

Tommy -- Roger that. That's one of those "when I get around to it" things...
Liked your story about hitting the scale house -- Reminds me of my trip to the ATHS regional show in Winchester, VA a c9ouple of years ago. I was driving a 359 Pete and had Mike Roache's '52/'54 2-ton and my '49 1-ton on the Landoll. Hit the scale house after crossing the US301 Potomac River bridge into VA from MD .... Longest red light ever ... right about the time I'm expecting to get the Park Bring papers sign, Freyja (who was riding with me -- a picture of her from that trip is currently my avatar ) sticks her head out the window and gives that classic German Shepherd "Stare" at the mirrored window ... How she knew to look directly at them, I don't know ... but I got the green light about 3 nanoseconds after that.
I guess they liked the dog
